This workbook will help you develop an Advance Care Plan that outlines your wishes about health care decisions in the event you are unable to do so. It will help you consider what is important to you, help you document your beliefs and wishes, make a Health Care Directive and/or appoint a Proxy if you wish, and help you make the Advance Care Plan that best suits you.
Feel free to move back and forth between the different sections. You can choose to answer or skip questions, and record your thoughts and wishes in each section.
If you want to save your workbook and come back to work on it, you can click on “Save and Continue Later”. You’ll be given a link that will be sent to your e-mail address – you can use that link to return to your workbook at any time for the next 30 days. However, if you wish to make changes to your workbook after those 30 days is up, you will need to start the Advance Care Plan Workbook again from the beginning.

If you want to make a legally binding Health Care Directive, you will need to complete a separate Health Care Directive, ensuring it meets the requirements set out in the Consent to Treatment and Health Care Directives Act. You can use your own format, or use our sample Health Care Directive form.
